We Welcome New Patients of All Ages

New patient visits need to be scheduled in advance by calling  (801)-302-0301.
Please allow an hour for your first visit which includes a review of your medical history, exam, evaluation, and treatment. The initial paperwork may take about 10 minutes to complete. If you prefer to download the forms and complete them at home simply click on the links below.

You can print and complete these forms at home and bring with you, or you can plan to arrive a few minutes early to your appointment to complete them in the office. Remember to bring your identification, insurance card, and a form of payment. All payments, co-pays, deductibles, and coinsurances are collected at the time of service.

What you can expect your first visit

The initial visit will include a review of your health history, physical exam, manipulation and therapies if Dr. Hall deems necessary. Please check in at the front desk and have your insurance card ready to scan. At that time we will explain your chiropractic coverage and collect payment. We do our best to determine what services are covered by your plan, but please keep in mind that verification of benefits is not a guarantee of payment. If services are performed, billed, and not paid by the insurance the patient is ultimately financially responsible.  

When you meet with Dr. Hall he will first review your medical history with you. Then he will hear your concerns regarding your current condition and then complete a non-invasive neurological and orthopedic examination. This may involve some testing of muscle tone, flexibility, range of motion, strength, balance, and sensory response. He will then recommend treatment options, and depending on the severity of your condition or pain he will adjust you at that visit.

Patients say that Dr. Hall is a good listener and takes the time to explain things to them. Through the different phases of care he educates and instructs patients on how to resolve pain, avoid future episodes, and prevent injury. After the patient has a reduction in pain and reaches phase two, exercise instruction and home care is discussed. Exercises specific to your injury, if done regularly play a big role in strengthening and toning the area, and preventing future injury.
